Elder Health Review

The in-home assessment focuses on key issues that are impacting elders and their families.  For example:

• Interpretation of care from diverse specialists. This will include placing these recommendations in the context of the patient's overall health and function
• Review of Medications with a Focus on Drug Interactions and Drugs that are Beneficial and Detrimental to the Elderly
• Evaluation of

• Loss of Function
• Memory Impairment
• Gait, Balance and Falls
• Safety Concerns
• Driving Concerns

• Education about Advanced Directives
• Evaluation and Education about What Options Are Available for Living Situations and Home Care

What you will recieve:

• A package with a written assessment of the evaluation, specific recommendations and a list of resources for the client and their families.
• A letter of evaluation and recommendations for the client’s primary care physician.
  

Fees: We charge a flat rate per visit and additional fees for any follow-up. 

Please note that our fees are not covered or reimbursed by Medicare or other insurance. We reserve the right to change our fees at any time.
