Improving seniors’ lives
Reducing stress for families and caregivers
The nationwide shortage of geriatricians limits many patients from accessing care from physicians with geriatric-specific specialty training and experience. In addition, the time constraints and productivity demands for primary care practitioners prevent many well-meaning physicians from having the time to fully understand patient-specific concerns and to evaluate the entire scope of the senior's physical, mental, and social well-being.
Elder Health Review works in collaboration with the primary care physician. After each encounter, with the patient's permission, a letter including a summary of the evaluation and recommendations is sent to the primary care physician.
Elder Health Review does not provide primary care services nor does it aim to replace the important role of your primary care physician.
