Client Examples

Example 1: Mr. and Mrs. S are healthy, active 75 year old seniors. They love skiing and traveling. They live independently and wish to continue doing so even if their health becomes compromised. They have planned their finances to accommodate this possibility. Now they wish to embark upon health planning.

Their assessment may include:

  • Recommendations for age-appropriate screening and preventive measures.
  • Education about Advanced Directives including a detailed discussion of what various interventions are and the likelihood of their effectiveness based on the current scientific literature.
  • Assessments of potential hazards in the home that can lead to future falls, fractures and injuries.

 

Example 2: Mrs. R has Alzheimer’s dementia and osteoporosis. She lives at home alone but her grown children are increasingly concerned about her safety.  In particular, they are concerned because their mother is still driving.

Their assessment may include:

  • A review of all medications (including those "extra" pill bottles in the medicine cabinet), with a particular focus on interactions and side effects.
  • Interpretation of care recommended by specialists. This will include placing these recommendations in the context of the patient's overall health and function.
  • Memory screening and care and coping strategies for her family.
  • Options in the community for driving skill assessments and/or available services for those who are no longer able to drive.
